India’s FDA drug inspections are back at record highs

FY2024–25 matched the 2019 peak. Roughly 1 in 10 drug inspections ends OAI — and about half end VAI, the documented “fix this voluntarily” shot.

After the COVID trough, FDA drug inspections of India facilities climbed back to the levels plant heads remember from 2019 — and stayed there. If your quality calendar still assumes a quiet foreign-inspection decade, the dashboard disagrees.

India drug inspections by fiscal year

202498151262759.5%
2025114152242908.3%
2026*45491310712.1%

FY2024–25 match the 2019 record. FY2026 is pacing similarly on a partial year. Read the middle column carefully: VAI volume is huge. Roughly 1 in 10 drug inspections ends OAI, and about half end VAI — a documented “fix this voluntarily” warning shot with open commitments to FDA.

India’s OAI rate vs peers

Drug inspections FY2023–Apr 2026, countries with ≥50 inspections:

CountryInspectionsOAI rate
Korea9214.1%
United States3,84812.3%
India9039.4%
Canada1787.3%
China4366.4%
UK836.0%
France1295.4%
Italy1323.0%
Japan1302.3%
Germany1690.6%

India runs ~15× Germany’s and ~4× Japan’s failure rate. That is the standing anxiety of every Indian plant head exporting to the US — and why continuous floor evidence between audits is no longer optional theater.

Scale of the India corpus

  • 4,662 FDA inspections at India facilities since FY2009 (#2 foreign destination after China)
  • 2,916 drug inspections across 1,191 distinct drug firms
  • 386 OAI outcomes overall; 301 of them drugs
  • 192 downloadable Form 483 PDFs at India FEIs
The wave is not theoretical. FY2025 alone delivered 290 India drug inspections — and the escalation ladder from OAI to Warning Letter is running hotter than ever.

Press nuance (calendar vs fiscal)

Business press in late 2025 and 2026 also reported calendar-year inspection volumes dipping (e.g. ~284 India drug inspections with outcomes in 2024 vs ~212 in 2025) and fewer headline OAIs even as surprise inspections expanded. Hold both truths: our fiscal-year tables show the post-COVID climb back to 2019 peaks; calendar snapshots can look softer while VAI and always-on readiness still define the job.
