An OAI is a coin flip on a Warning Letter

An Official Action Indicated classification is not the end of the story. In the India drug data, it is often the start of a visible clock: Warning Letter or not, within months.

The escalation rate

• 51% of India OAI drug inspections (153 of 301) were followed by a Warning Letter at the same FEI within 18 months
• Median lag: 193 days (~6.5 months)
• FY2025 (23) and partial FY2026 (19) are the two heaviest India drug Warning Letter years on record (previous peak: 22 in FY2020)

Cohance Lifesciences (formerly Suven) shows the ladder in exchange filings and press: August 4–12, 2025 GMP inspection at Nacharam FDF Unit-I → Form 483 with six observations → OAI classification → Warning Letter (FDA letter dated January 30, 2026; company and The Hindu disclosed in early February). US revenue from the unit was described as under 2% of consolidated FY2025 sales — small financially, still a public quality event.

OAI isn’t the punishment. It’s a coin flip on a Warning Letter with a visible clock — and the corridor already shows how the same facilities climb the ladder.
